Consultant for Title IV, SDFSC
SDFSC Special Programs Request
A Region 14 ESC Title IV SSA member district may request speakers, programming, and materials to augment their Title IV SDFSC efforts. To make these requests the following steps must be followed:
Step 1: The request must be allowable under Title IV allowable expenses, meet the Principles of Effectiveness (provided below), and not break the “Red Flags” of Prevention (provided below).

Step 2: The request must be made on a SDFSC Co-Op School Request Form (provided below) and approved by the SDFSC Consultant before the programming will be paid for.

Step 3: A Speakers Intent Form (provided below) may be needed depending on the type of service being requested on the SDFSC Co-Op School Request Form.

Step 4: After approval, the district will pay for the programming; and after providing billing statements to the Region 14 ESC SDFSC Consultant, the district will be reimbursed for the requested expense.
